Chaptership Terms Form: Each officer must sign this form by hand. This form represents a chapter’s commitment to FIMRC’s vision and mission.

Officer Information Form: Forms must be completed for each officer. This form is used to gather important contact information to ensure that our team remains in regular touch with each officer.

Semester Report Form: This form must be submitted at the end of each semester. Our team uses the information in these forms to provide formal recognition of your chapter’s hard work
